应对方式对癌症患者抑郁情绪和生活质量的影响

摘要： 目的探讨应对方式对癌症患者抑郁情绪和生活质量的影响。方法住院癌症患者完成一般资料问卷、抑郁自评量表(SDS)、医学应对问卷(MCMQ)、中国癌症患者生活质量调查问卷(QLQ)调查。结果符合条件的234 例患者中，54.27%伴有不同程度的抑郁情绪。MCMQ面对因子(r=-0.378, P=0.006)和回避因子(r=-0.374, P=0.008)与抑郁情绪呈负相关，屈服因子与抑郁情绪呈正相关(r=0.684, P=0.000)。MCMQ面对因子(r=0.232, P=0.043)和回避因子(r=0.317, P=0.000)与生活质量呈正相关，屈服因子与生活质量呈负相关(r=-0.479, P=0.007)。分别以抑郁程度和生活质量为因变量进行多元逐步回归分析，回归方程分别为：Y(抑郁程度)=-0.631+0.161X3(屈服)，Y(生活质量)=3.317-0.085X3(屈服)+0.257X2(回避)。结论应对方式是影响癌症患者心理和生活状态的一个重要因素。

Abstract: Objective To explore the effects of coping styles on depression and quality of life of cancer patients. Methods Hospitalized cancer patients were investigated with general condition questionnaire, Self-rating Depression Scale (SDS), Medical Coping Modes Questionnaire (MCMQ) and Quality of Life Questionnaire for Chinese Cancer Patients (QLQ). Results 234 cases were selected, and 54.27% of them accompanied with depression. The factors of confrontation (r=-0.378, P=0.006), avoidance (r=-0.374, P=0.008) and acceptance-resignation (r=0.684, P=0.000) of MCMQ were related with the depression, as well as the quality of life with confrontation (r=0.232, P=0.043), avoidance (r=0.317, P=0.000) and acceptance-resignation (r=-0.479, P=0.007). Multiple stepwise regression analysis showed that: Y (depression)=-0.631+0.161X3(acceptance-resignation), Y(quality of life)=3.317-0.085X3(acceptance-resignation)+0.257X2(avoidance). Conclusion Coping styles are related with the depression and quality of life of cancer patients.
