颅脑手术麻醉后恢复室常见并发症

Postoperative Complications after Intracranial Neurosurgery in the Postanesthesia Care Unit

摘要： 目的调查颅脑手术患者在麻醉后恢复室（PACU）并发症的发生情况。方法对2009年3月～12月进入PACU的颅脑术后患者2166例进行回顾性分析。结果常见的并发症有高血压（14.0%）、心律失常（13.9%）、疼痛(13.7%)、寒战(10.5%)、恶心呕吐(9.3%)、谵妄躁动(8.6%)。低氧血症(3.4%)、呼吸道梗阻(1.9%)、苏醒延迟(1.5%)、低血压(0.5%)的发生率较低。1例患者发生误吸。并发症总的发生率为52.2%。在监测体温及肌松残余的患者中，体温异常和肌松残余的发生率分别为7.2%和22.8%。全部病例经PACU治疗均恢复正常。结论神经外科PACU的常见并发症有其特殊性，需要针对不同的病种、手术严密观察及时处理。

Abstract: ObjectiveTo investigate the complications after intracranial neurosurgery in the postanesthesia care unit (PACU). Methods2166 patients enrolled into PACU after intracranial neurosurgery were reviewed. ResultsAmong the complications, the most frequent ones were hypertension (14.0%), arrhythmia (13.9%), pain (13.7%), shivering (10.5%), nausea and vomiting (9.3%), and delirium (8.6%). The least ones were hypoxemia (3.4%), respiratory obstruction (1.9%), delayed recovery (1.5%), and hypotention (0.5%). Abnormal temperature and residual block occurred at 7.2% and 22.8% in the patients who were monitored. All complications were treated immediately, and all the patients transferred to neurosurgical ward with modified Aldrete score over 9～10. ConclusionThe patient in PACU need well management to insure safe and smooth recovery from anesthesia after intracranial surgery.
